The following forms must be completed in order to be an ARPF volunteer (mile stewards, oak grove stewards, frequent work group volunteers): 2013 Annual Waiver of Liability(1) Youth Waiver of Liability(if under the age of 18). NOTES:
1. The 2013 Annual Waiver of Liability should be completed by those who don’t wish to fill out a waiver of liability each time they participate in an ARPF activity.  This waiver must be submitted with an original signature, and will be kept on file until December 31^st, 2013.

2. The Individual Event Waiver of Liability should be used by any individual participating in fewer work groups per year.  This waiver is kept on file for a shorter duration and only pertains to a specific day.  This waiver must be completed at each event by anyone who has not completed and filed the 2013 Annual waiver.
